Immigration Physical

The immigration physical exam procedure is one which requires an experienced medical team. Dr. Jean R. Charlot is a Bureau of Citizenship and Immigration approved Civil Surgeon. He and his staff have been performing Immigration Physical Examinations for over 4 years. We have assisted hundreds of patients to obtain the medical clearance they need to become a United States resident.

What do you need to bring to our office?

• Your immunization history to be reviewed by our medical staff
• A valid United States/State issued picture ID

Immigration Physical exam and Immunizations:

Adults with all current immunizations: $90

Children under 18 years old: $70 which includes immunizations

Adults without current immunizations $380—which includes:

• Complete Physical Exam
• Immunization Status Review
• Administration of Required Immunizations
• HIV and Syphilis lab testing
• Tuberculosis testing
• Typed/Completed/Sealed I-693 form
• A personal copy of I-693 form and lab results

Please note:

• Depending upon test results and past immunization information additional services may be required. Commonly, a chest X-ray or additional blood work may be required—and can be completed in our office— at an additional cost to complete your application.

Federal Guidelines

1. All applicants age two and older are required to be tested for tuberculosis. To ensure proper reading and diagnosis, the patient is required to return to our office two days after administration of the test. If this test results in a positive outcome, a chest x ray is required to confirm diagnosis. The chest x ray will require an additional cost.
2. All applicants age 15 and older must be tested for syphilis and HIV.
